Creating Your Client File 

We are excited that you chose Party Masters Events + Services for your event. To get started, so we can create your client file and send you an agreement, and input your event details in our booking system, below is the information we will need from you. 

Once you complete the questions below, we will send you the agreement to review, sign and return, plus send you a link for the payment, to confirm the services on this event date. 

Please complete ALL areas, as best as possible. Some may not pertain to your event, which you can leave blank.
